On the day of the procedure, the patient, or responsible party, should:
- Review all information listed on the patient’s chart, such as spellings, addresses, other demographic information, for accuracy.
- Ensure that the patient has a ride home and a responsible adult at home to care for them that evening. For patient safety, we request that a responsible adult remain at the center with the patient at all times.
Each patient, or responsible party, should bring the following on the day of his or her procedure:
- Insurance card(s),
- A photo ID,
- Form of payment, for any copays or balances, if required.
Pediatric patients (17 years of age or younger) must have parent(s) or legal guardians at the center at all times until discharge. Legal guardians should bring any legal or guardianship papers to the center with them on the day of the procedure.
